The Dr. wrote it on his script pad and yes i kept it- no date but it is his writing. This is so bad but my first thought was ... well then... cuz he didn't send you thry the proper channels right away...then....toss a date that script then..but that is bad of me huh?? Okay so possibly since you have it you can contact them asking to look back in your records and match up you visit with the date he wrote it...can you think of at least the month or some event etc...that will jog your memory?? Then have the guy fix it... whatcha think??
