I am really sorry about what you are going through right now.
Anyway - while I'm no expert - I would think that you would be better off in the long run if you took all of the sick time that you are eligible for, and then a medical LOA from your work (which will require a doctor's report), and finally - documentation of your inability to return to work after your medical LOA expires.
This will establish that you did not voluntarily leave your job - - but rather you are medically not able to work even after exhausting all of your options. This becomes very important when / if you apply for any type of assistance.
You'll probably end up in the same place - ie. no job and fighting for SSI (if you really are denied SSDI which I would still apply for no matter what) - but at least you will have both medical and work history documentation that supports your case.
Good Luck with this - it can be so time consuming and stressful - especially when you aren't feeling well to begin with!!
