I know I've renewed mine in the last few years and I don't remember having to answer any medical questions. I did, however, almost lose my license because my doctor threatened to pull it!
That was at the beginning when I could hardly walk with a cane. My right foot was weak enough (foot drop) that he was worried that I couldn't pick it up fast enough to go from brake to gas and back. I really wasn't having trouble unless I was tired or drove too much. We made a deal - I wouldn't drive anywhere other than necessary, and not if I was tired, and I would get PT.
The PT agreed with me that I could drive with limitations. Between IVSM and PT, I regained most of what I had lost, but I was scared for awhile that I wasn't going to get better and that I would lose my freedom.
Personally, I think that it is between us and our doctors - not the DMV. I do know that my docs won't hesitate to yank my license if they feel I'm unsafe.
