Does your dr know the meds aren't helping and lack of sleep is a big problem?

I think most with any kind of chronic pain end up with having a long acting or time release meds for base pain relief, then a quick acting med for flare ups, and usually an anti depressant {also works with pain meds for better results} and even a sleep help med.

Sleep is very important for the body!

If your dr doesn't seem to understand all this or be open to helping you , might be time to find a better one.
