THIS eval makes SENSE ..... mine had like a thousand questions (a bit exaggerated ) and they asked all those stupid questions....like "did you ever enjoy killing small animals when you were younger?"

This eval really targets PAIN issues!
These are questions a PM doctor should ask EVERY patient.

I think this is fabulous and THANK YOU so much for sharing it! I know a lot of people will benefit by reading these and we can refer new patients to look at it.
