If it's swollen, it makes sense that you would feel it at ALL settings as the leads would be pushed closer to the spine. I don't understand their thinking. They're just giving you a line of B.S. cause THEY don't know what's wrong.
Keep bugging them - they'll get tired of it and do something about it.
