We've put all our meds/supplements (along with pharmacy & PCP info) into a spreadsheet (any type of table will do) and just update it and print off a copy the night before a visit. Then write "see attached" on the form they give you. We have columns for med name, dosage, frequency, and notes (including reason for taking).
Saves time and reduces the stress/redundancy...

I've been doing that for the past 10 years, but in about 1/2 the cases they don't want to attach it, they want it written in the proper box
(or line, or .....whatever!) on their sheet.
