I'm going to Freischlag currently and she'll be doing my surgery on the 5th. I posted her Baltimore address info in the board listing doctors that deal with TOS. She'll evaluate you to see what route would be best for you. For me she pushed surgery but that was after my diagnosis injections and botox injections worked so well for me. I'm guessing that if they hadn't she would have suggested a different route (tho conventional PT wasn't doing diddly so I'm not sure what route would have been left other than repeat botoxes).

There's also a Dr. Bonnie Gerecke at U of MD Medical in Baltimore that might be able to help. I'm not certain but I don't believe she has as much experience as Freischlag but she'd be able to definitely help pinpoint whether you have TOS or not. I was going to undergo a specialized EMG that U of MD Medical developed for TOS but my symptoms shifted enough that I didn't need any more convincing that surgery was my best route. Hope this info helps. After my surgery next week, I can give a better idea of how good a surgeon Freischlag is and how good/bad a recovery it is from the type of surgery that she prefers to do. Fingers and eyes crossed it'll be as easy as my C5-C6 fusion.
