Default How many stages are there to be approved by SSDI doctors?

I'm not even sure how to ask this without making this too confusing. My husband applied in person for SSDI at our local SSDI ofice.

They the local SSDI office said all the info was sent to the Capitol of our state of Nevada.

Now the caseworker from the capitol told us that their doctor will look over my husbands info next week to see if he will be considered disabled. Now at that point I figured the case is over. But our case worker says after a Nevada SSDI doctor makes a decison then San Fransico CA has to make a decision. Thats where I'm confused.

So i guess one of my many questions is, if the capitol ssdi doctor says my husband is disabled, does San Fransico CA usually agree? Or is it 50/50? And what exactly is in Sanfransico Ca regarding SSDI? Is it a hub? I thought the info went straight to Baltimore MD.
