I don't have the answer for you. ButI have to say I leaned heavily on the book California Workers' Comp: How to Take Charge When You're Injured on the Job By Christopher A. Ball while I was going thru the various systems. Maybe if you scan through the book online you can find the answers - it's pretty much plain English. Here is the link (click link for the Google Book)
I wish I had the answers for you readily. I'm a little overwhelmed these days - otherwise I'd help research this better :/
