If malabsorption is due to the most common cause (insufficient stomach acid) and is not severe, B12 in vitamin form can get through but the body cannot break it out of food.
If malabsorption is severe (loss of intrinsic factor), the very large dose )1000 - 2000 mcg all at once per day is standard) allows enough to get through by default to begin building up stores.
