So why does she have this deficiency? What else is wrong with her?

If her tests are good, obviously the cyano is working.

Some people may become energized on B12... and subjectively report,
it as "anxiety". But we are all so different. There may be other factors she is not telling you... like the caffeine, or RX drugs she may be taking, etc.
