Even though I am Canadian, I thought I would post a link to the Workers Compensation Board for B.C. and it's guidelines for CRPS. I have been studying this manual as I am battling WCB right now as they are cutting off my physio therapy and trying to take away my home Tens Unit.

Sorry if it is not helpful for you. Thought it might be interesting for you to read one Insurance companies guidelines regarding CRPS. I know Manitoba WCB also follows these guidelines as well.
