What I know: Automatic refills are done by computer.
A human does not see them at all until the request pops up on the pharmacy screen.

A well run automated program, is supposed to call you when the refills are expired. But it appears yours did not have that in its program software.

Many states have time limits on refills. For non controlled subtances, in many it is 12 months. Controls have a maximum of 5 refills within 6months, and that is a Federal rule, and covers all states. Some states like NY may be stricter with this.

When you typed-- Clonazepine-- did you mean Clonazepam?
Or Clozapine?

Clonazepam is a controlled substance and would be in the
5 x within 6mos rule.
Clozapine is not, but would not be in an automated refill program for other reasons. (it requires a blood test before every refill).

The Elavil would be in the 1 yr refill bracket, if the doctor ordered it that way. The 1 yr rule is for people on long term therapy, but any doctor order less than 1 yr would be entered as such. Say if your doctor wants you in for evaluation every 3 or 4 months or whatever. So it really depends on what your doctor ordered in the refill field.
