That is something you have to discuss with your insurance company. I believe they are all very different.

From the postings here in the past, only a diagnosis of diabetes, will get you special shoes on insurance for "most" of them.

There may be an appeal process for others, to use, and that would be spelled out by your plan. In those cases, the doctor sends in diagnosis and reasons to the insurance for evaluation.
