--I'd consider either trying to get an appointment with the Neurology department at Johns Hopkins in Baltimore MD, or come north to the Cornell-Weill Center for Peripheral Neuropathy in Manhattan. Both have combined neuropathy specialty practices with major research programs into neuropathy causes/treatments and the doctors you will see are clinician/researcher hybrids, so they are much up on the latest in the field and on proper testing procedures.

You may have to check to see who takes your insurance at each place, and there do tend to be fairly significant waits for new patients, though the latter can be somewhat mitigated if you're willing to call in about cancellations every so often (especially at Cornell).
