Since you mention joint pain with these other autonomic symptoms, I would suggest you get worked up for autoimmune disease. The diagnosis process ahead can be long and frustrating. Find a good rheumatologist to do blood work, then go from there based upon the results. Keep in mind several AI diseases can be present even when blood work is negative (called sero-negative).
Do you have dry mouth or eyes? What about fatigue? The palpitations and SOB sound like more autonomic dysfunction. Have a cardiologist order a 24 hours holter monitor or 30 day event recorder so you can document these events.
