I believe the problem is that the scalenes sit under the sternocleidomastoid. You have go through the SCM. I believe ultrasound or fluoroscopic guidance is required for accuracy. Otherwise, you are just dry needling the SCM or risking going through the scalene to far. Dry needling would require the same setup as a scalene block so you might as well do the block.
Just my non-medical opinion.
