Sad to hear of your situation. I hope this helps you in some way.
When I signed up to use angelflight they didn't require me to bring anything but a note from my doctor.

I ended up not using angelflight because my caretaker didn't want to fly home alone with the pilot over the rockies.(coming back from Denver)They were going to fly a small plane. I have heard that sometimes they give you airline tickets.

There is some info on getting free airfare from the airlines such as Southwest, etc.
