When I signed up to use angelflight they didn't require me to bring anything but a note from my doctor.
I ended up not using angelflight because my caretaker didn't want to fly home alone with the pilot over the rockies.(coming back from Denver)They were going to fly a small plane. I have heard that sometimes they give you airline tickets.
There is some info on getting free airfare from the airlines such as Southwest, etc.

PatientTravel.org and its related telephone HELPLINE (1-800-296-1217) serves to make referrals and provide "how to get there" assistance for any patient needing to travel to a distant specialized medical facility for evaluation, diagnosis or treatment. Patients served are people for whom the cost of long-distance travel for medical care is difficult financially.
Angel-Flight.org is a non-profit charitable air medical transportation organization serving needy people and their families. We provide access to specialized medical evaluation, diagnosis, treatment and rehabilitation.
AirLifeLine The Methodist Hospital is pleased to partner with AirLifeLine to provide free air transportation for patients and their families who are in need of transportation for medical treatment.
