You have them in the wrong order?
  1. Neuro. huge increase migraines - number and severity
  2. Pulmonary. Follow up exam for recurrent/episodic cough
  3. Gyn - been so long blah blah blah just like the rest of us. LOL
  4. Dental - possibly problem with recent reconstructive filling
