For me, the lower end of the painscale is harder for me to define, A handful of times, I've had very bad "runaway" pain that made me nauseous. I would rate that a 9 on a 10 scale. Twice, I've had pain where I literally passed out and woke up in an ambulance. That was way at the beginning of my condition, before I learned how to manage with medication.
I guess I'd call a 7 a bad pain level that requires a trip to the emergency room.
