The pain I get is very similar to what you can't describe! 
I just figured it was from a combination of things, and since I can't spend a lot of time on my feet anyway, it hasn't been as pressing a problem for me. I can definitely appreciate how it could/would be though...

Mine, was custom-made too. There was a stall at Sam's who sold this. I forgot the company. its not a Sam's product, it was one of those temporary products/companies allowed by Sam's to "park" in their premises. Anyway, I bought it for $190. It absolutely did nothing!!!!
I tell you, Doc, I also bought that gel-like products from the pharmacies sold by a popular brand (I am not sure if I can mention the brand), well, I am not sure if its helping, but it is better than the supposed custom-made sole-insert. The expensive insert was creating more pain than relief!!!
