Hi facepain,

The throbbing pain above the area of the extraction can be caused by any number of things. It does indicate inflammation is present. How far up the problem is, will hopefully be seen on the scan.

The gum closing over the surgical site does not always indicate complete sinus closure. It's like having a splinter in your finger and the gum closes over it. The only way to see under the closed skin is to take an xray or scan.

Sometimes the infection in the sinus travels upward and this causes a void in the bone. This is most likely what your ENT dr is looking for.
