One cannot go just by imaging, clinical findings, labs and history are important too! On imaging, edges are usually ill-defined in more aggressive lesions. Cysts are usually well-circumscribed and are brighter with certain MRI spin echo times. Biopsy is definitive. It is much more complicated than this but this is a simple start...
