Are they very sure the PN/alcohol is the cause for the clawing?
I ask because in the past some with thoracic outlet syndrome (TOS for short) have had a nerve getting pinched or compromised, that caused the hand to claw. Usually the nerve was affected up near the neck area.
Have you had a MRI of the cervical spine to check for issues there? Just to be sure.
