Vitamin D2 given on RX really does not work well. New studies show it is 87% less effective than OTC D3 forms.

I'd try epsom salt soaks for that foot. Often there can be low magnesium present which upsets calcium metabolism as well.

Do you know your actual D3 test numbers? This would help you dose yourself properly to bring up low readings.
The general guideline is 1000IU of D3 for every 10 points needed for raise. This varies however from person to person. People with more fat on their bodies may need more.
