For myself, listing symptoms with the worst as # 1, and then #2 and so on, works the best for me.
Or start at the top of your body and go down with the symptoms/pain, which ever way works best for you.
I think for a dr a list like that would be good too, instead of large paragraphs, just give one symptom or issue per line/number and they follow up later by asking you about it.
I would think they have to ask and document it that way, instead of just going by a list someone makes at home.
They have to look at you in person and hear the details they find relevant , then if there is some other testing or exam things they can use that information also.
