Unknown if you can or not. Go to the SSDI website. A quick review - I do not see any forums for doctors. Appears the forms are for the "claimants." I gather the doctor forms are snail mailed and/or faxed directly to the doctors office.

As for your letter, it should be as thorough/detailed as possible. Don't worry about the letter being long. Just make sure it is "typed" or neatly printed, has paragraphs, no CAPS (considered shouting) and has paragraphs.
Or better yet, set up a "conference" appointment with your treating physicians.
On your own, make cliff notes so you won't forget anything that you want to talk about.
