The "list" should be on a separate piece of paper. The doctor will more than likely read the letter at her leisure if you are there for an office visit rather than an appointed conference.

NOPE! The reason is because, unless you have your own copies of your medical records and doctors notes, you do not know what information SSDI knows about you.
Don't worry yourself sick about this. As long as you are truthful, your treating physicians are on your side when dealing w/SSDI, you have nothing to worry about.
