Little about me. I got my first Disability update form ssa-445-ocr-sm this month and my question is about the 3 most recent doctor visits. since feb 2011. Do I put down that I saw the doctor for headaches or heartburn issues? I made a visit with the ENT for my ear pain and pressure caused by the nerves.
My deafness is never gonna get better and I'm sure it will get worse sooner or later, Do I need to put that in the form or SSD already knows that my problem will never get better.

put down the last 3 visits pertaining to what you were approved for and under reason for visit just write the condition that you were approved for that the visit pertains to. dont write in the remarks section at all.
