Middle finger is median nerve. This could be caused by TOS, cubital tunnel, or carpal tunnel. TOS generally includes the ulnar nerve (4th and 5th digits). Maybe an EMG/NCV study would be indicated. Find some median nerve stretches/glides and try those. You may need to find a different PT as they should have tried treating you for carpal and cubital tunnel.
