Athena Diagnostics are the only ones who test for most all of the types of CMT. And there are many and if you have to test for all of them it is very expensive. They have a patent on them.
www.athenadiagnostics.com. You can give them a call and they are more than willing to answer any questions you may have.
They told me that Baylor University out of Houston, TX tests for some of the types of CMT. However, they use a different methodology (way of testing) than Athena Diagnostics.
