Hello Everyone,
I promised I would post my results as I went through each test phase to determine the cause for my wavy vision problems. This isn't specific for vision, but may be cause for the symptoms I am encountering. Here goes:
History: Dizziness
Interpretation: The patient has a normal tracking test. He had optokinetics both to the left and the right at 40 degrees in each eye. He had no nystagmus noted during the testing. The patient had difficulty keeping his eyelids open. (Personal note: first time I have ever had an ocular MG symptom that I was aware of) His saccade testing showed an abnormality in the vertical plane in the right eye. His gaze testing was normal, except for the vertical nystagmus in the left eye looking straight ahead. He had a mildy abnormal Hallpike maneuver with no nystagmus noted.
Impression: STRENGTH
1. Mildy abnormal CENG with abnormal optokinetics at 40 degrees and abnormal random saccade testing in the vertical plane in the right eye. Hallpike Maneuver was mildly abnormal without any nystagmus noted. This patient likely has benign postural positional vertigo, however, I recommend an MRI to evaluate any other etiologies causing his dizziness. He had no noted nystagmus during his testing.
This is all I have for now. I'm not 100% sure what this means because I have been having wavy vision problem. I am praying to God that this is my MG acting and causing the wavy/distorted vision. I see the Neuro-Opthamologist Wednesday. I am bringing my completed Neuro history along with this documentation and the result from the Optician. Just a note, DO NOT see an optician for significant eye problems. He told me to basically wait 4 weeks to see if the glasses I ordered helps or not. He didn't bother to dialte my eye and said everything looked great. I'm not putting him down, but he should have did more for my exam when I told him I was having a problem and that the lens he offered weren't fixing it.
Follow blood work:
Phenytoin/Dilant <1.0 Normal
Free T4 (Thyroid) 1.10 Normal
TSH 3rd Generation 1.387 Normal
Sedimentation RA 9 Normal
Antinuclear ABS Direct *Negative = Normal
Vitamin D 28.5 Abnormal normal range is 30.00-100

B12 690 Normal
ACHR Binding Antibodies (MG) .45 High normal is .00-.24
One thing I am confused by is that the test for MG in 2008 for ACHRAB was 3.62 with being under .3 as normal. The other ACHR test (which I am assuming may be different) was still high but at .45.
Stay tuned everyone, I hope this helps someone. I am going to look into this eye testing to see if it's the cause of my symptoms.
