I too had a vitamin D deficiency and it contributed to me having peripheral neurothapy. I will say that I have improved since I quit the prescription they gave me, and went and bought over the counter D3. It seems to be much more effective for me.

Anyway, thought I'd post something that might be helpful?.

I was recently diagnosed with a severe vitamin D deficiency. My symptoms have been very much like PCS - dizziness, debilitating fatigue, inability to think straight, spaciness, headaches, aches and pains etc. Initially, I thought that the PCS had come back. I'm now on a three month prescription dose of D3 to bring my levels up to normal.

So if you are able to request a blood test (not sure how it works in the US) it could be worth getting your vitamin D levels checked. It seems to me that having the correct level would help some of the symptoms perhaps?
