10 pain is very close to passing out pain, that is not a good thing, might be best to go to ER.
If someone is there with you to keep watch over you then maybe you could wait. But not if alone.

Could be something new or something that is treatable and not only TOS pain.

It isn't RSD/CRPS pain is it? Your TOS surgery might have triggered RSD/CRPS, it has for others in the past.
