I have got no idea how the health system works in the United States, but in OZ when in need of treatment in a emergency, which it is in your case with the very high pain levels, people are taken to Casualty, i think yours is called ER, the most urgent cases are of course are seen to first, but i would think seriously about heading to a big teaching hospital [ during the week, no good on week ends] but there should be specialists crawling all over the place, that one of the hospitals gp's could call upon for opinions, you may get the ball rolling at least , and i would take in any prior tests and reports.
