After reviewing this evening, the first half of the report needed very few edits, the second half, while readable needs quite a bit of work. Some thirty pages, not counting references . . . so I avail as prototype and welcome any comment or suggestions. It will likely become 20% to 30% longer.

the first reports one ALS patient weaned from trach after pacer implantation, the second and third address the diaphragm thickness issue, otherwise only reported by Onders at this moment.
