Try to find an endo that is a neuro-endocrinologist so that they will look at pituitary issues.
TSH in the middle of the range is icky feeling, you need to be at the bottom to right there, your doc is an @$$. I am not sure about the other symptoms - if they are related or not as they sound more ENT or neuro - like TMJ, migraine, etc.
