Default Pain and Med tracking

I am looking for something that may not exist..... I am looking for a Chart that will help me to record how I feel (symptoms) on a daily basis so that I can show this to my doc. Does anyone know of such a chart? I would also like to find a chart that will help me to list all of my medications and the side effects of each throughout the month.
Any, and I mean any suggestions would be appreciated.

I think this is a very good idea. Any journal keeping can only help you to explain your symptoms/results to your doctor. It can also help you find trends and target improvements or side effects.

Hope you find the chart you are look for, it can be for your advandage to keep information so your doctor can work with you. Great that you thought of this.

I have been searching for a chart to track my symptoms and meds on a daily basis. I have found a couple charts that may be of great use to some of you. These are nice charts.

I'd suggest you get an A4 diary, say, with 3-4 days each page, with times shown. Called a Week to View.
This is what I use.
The times are handy reference points for medication taken, and there is plenty of room for comments.
There are also yearly charts that could come in handy.
I tried PC based recording, but the last thing you want to do when ill is work on a computer, so the details get omitted.
A diary is very simple and you can keep it close by.
I doubt that you would find anything else, without it being too complex.
Keep it simple and you will use it.
