Confused osteomyelitis/osteonecrosis

hi

i have dental pain in the area of teeth #30-32 for the past 10 years. first had the 2 remaining molars 30/31 rc'd then had apico's. then had upper molars rc'd and apico's...still had pain....finally bit the bullet and over 2 year period had all 4 molars extracted...still have same pain in original area of lower jaw...have had this area dbrided several times...each time dead bone and "gunk" was removed. (wisdom teeth were all removed in my teens)

had ct cone beam done 3 months ago...imaging revealed bone fragments, root tips and perhaps infection.

i've had work down by top notch os's over the years...but obviously something has been missed.

any advice on who is should see to help me end this pain once and for all.

willing to travel anywhere for the best care.

Hi Lisa,

The conditions you are dealing with are very, very difficult to erradicate. This is similar to what I was discussing with Jasmine on a different post. The longer the condition goes untreated, the more the jawbone deteriorates from the bacteria embedding itself deeper and deeper in the bone. Chances are that your condition began from the wisdom teeth.... either there was infection there that was not removed and/or the ligaments were left in the bone at the time of the extractions. So if that is the case, then it's been brewing for many years and it may not be able to be erradicated completely. But at best, the dentist should be well versed on THIS subject and very experienced in dealing with it.

This problem is more common than it is acknowledged in the traditional dental profession. It often goes misdiagnosed and the patient undergoes repeated root canals and apicos leading to eventual extractions. None of which take care of the original problem and actually create a bigger problem. Surgical debridement of the areas is an attempt to erradicate as much diseased bone and tissue as possible. However, the removed pieces should be biopsied and cultured to determine the proper antibiotic protocol. Was any of that done during any of the debridements?

What is very disturbing to me (besides the whole ordeal!!) is the pathology that showed up on the (post op) CT scan. After all of the procedures you've had done in those areas, there should be NO bone fragments and NO root tips whatsoever! If there are, this clearly indicates that you have not had thorough extractions or debridements. Bony fragments and roots from extracted teeth sitting in the jawbone will harbor infectious bacteria and will cause additional deterioration of the jawbone which will only add more problems in an already compromised area of your jawbone. Certainly these things should have never been left behind in the first place, never mind after all of the surgeries you have had to remove the infected material.

I would go to the IAOMT.org website and seek someone who belongs to that organization who is very familiar with treating osteomyelitis and necrosis.

Have you ever been on IV antibiotics for this condition? Were meds prescribed for you at the time of the surgeries? Were any biopsies or cultures ever taken?

Lisa, before you allow your dentist to do another procedure, please check with the IAOMT organization and see if they can help you.
