Confused Lower extractions, Implant posts placed same day & then informed of massive infection

Hi, On 10/10/12, I had all my lower teeth extracted by an oral surgeon who, at the same time went ahead & placed the implant posts for my lower denture that will serve to hold it in place. After the procedure (I was put to sleep) I remember hearing him tell my daughter that once he was in there, the infection was massive & had even gotten into my jawbone. Told to continue on the antibiotics I had been prescribed the day prior to my procedure & rinse w/ the blue mouth wash. I went back 1 day before my check up visit was regularly scheduled b/c The post on my left side was showing a bit. After seeing the oral surgeon, he said it was fine b/c he would normally have left the top of these posts showing. (I assume he closed mine due to infection & wanted to prevent anything else from getting in..??) I was told he had done a bone graft b/c of the "massive, massive infection he found". He put me on Clindamyacin & sent me home. Now it's the 20th, I am still in lots of pain...I've never put my lower dentures in, he said to wait, wait, wait...and give my mouth & all that infection time to heal. My worries/ concerns are.. His 3-D images didn't show up this infection, or any bone loss..I have absorption problems, of which I made both him & his staff well aware of prior to any treatment, I am already on fentanyl patches & other narcotic pills for other health issues, yet I am feeling this amount of pain. I am worried the infection is still there, he should not have done post placement that day, Maybe I should've been given IV antibiotics...and the list of my worries goes on & on...plus now, even more of the post is showing & it seems where I have sutures along the top of my gum line where that post is showing, is trying to also open up...My oral surgeon, according to his assistant as of 10:15 this morning was on a plane travelling across the country & she was going to contact him & get back with me ASAP... It's now 6pm & I have yet to get that call... HELP... I am so worried....oh and to top everything else...B/C of other health issues, I'm not sure that Clindamyacin was exactly the best choice for me & my body! So, now my head is really spinning!~ thanks for any help, advice, etc. anyone can give!! ~

Hi cb,

I think your surgeon may have been a bit over zealous with your treatment. It is definitely not ideal to place dental implants in bone that is infected. The fact that the gum tissue is opening up near that exposed implant may be a sign of infection. The pain you are experiencing is also a concern, especially since you are on heavy pain meds for other reasons.

The antibiotic clindamycin is often prescribed for dental surgery. It can however, cause various degrees of intestinal upset in some people. Sometimes this antibiotic can lead to an intestinal infection called Clostridium Difficile (C-diff for short) which often requires IV antibiotics usually administered in the hospital. If your side effects are more than just slight or they get progressively worse, call the pharmacist and get his advice on what to do.

I think you need to be seen within a day or two. If the oral surgeon is not available, I would suggest that you consult with a different oral surgeon. He has to have someone on call for him. It is also best to bring the scans with you.

In the meantime, rinse with warm salt water 3-4 times a day to help keep the bacteria down.

I wish I could offer more help.... but your situation cannot be helped here on the web. You need to be seen by the surgeon.

Thanks for your input! I thought he was being so myself! & I had even wanted to wait. I had planned on having the extractions done & waiting which would have given everything time to heal before I went back to get the posts implanted. However, the day I saw the oral surgeon for a consult so he could get his X-Rays & see me before the extractions were to be done that afternoon....The oral surgeon actually called my general Dentist & said he would feel better doing the extractions & placing the post at the same, so it would be LESS stressful, traumatic & painful on me, speaking in the long term...And he made my appt. for the very next day. (I was suppose to be going out of the Country to visit friends, for a month. Not now!) I was ok to wear dentures, wait, go back later, etc. So...Since the infection did not show up on any images... When do you think, generally speaking of course, he would/could/might have realized just how involved the "massive infection" was? After the extractions or after drilling the posts into my jawbone? And when he did realise, in your opinion, would it have been best to have stopped & waited? ( I mean, I had already paid him in full, so that couldn't have been an issue). The problem I have also have is, I made sure he & his staff were well aware that I have absorption problems, I had gastric bypass in 2001 (& wish I could go back...but..) So now, I have a very small stomach that IS ALWAYS irritated along the line where the staples are. ( My gastroenterologist has informed me after each endoscopy that it is always inflamed to the point it looks as if it could bleed) I have recurring bleeding ulcers & take nexium 40mg twice daily & I am suppose to take carafate 4 times daily, after each meal & before bedtime, but it has gotten to where it gags me & worse, so much, I no longer use it. I have been taking Vit D., 5000iu, for about 3+ yrs due to the absorption problems, I take a probiotic also, it simply states its lactobacillus acidophilus-100 million at time of manufacture...which can mean...it's beans by the time it gets to me..?? My gums are so sore it hurts to touch them with my tongue and if I bend over it feels like I have a lot of pressure w/in my lower jaw & gums... So, should I wait until Monday's appt. to see the oral surgeon (of who I'm losing faith in, not that he hasn't been super nice or seen me/ answered calls, w/ the exception of today...but b/c...I feel like if I, not being a medical professional have enough common sense to think/know/figure out he should've waited...b/c in doing so, would've been in MY best interest, he surely should've known so as well & should've acted accordingly). Or do I need to go on to an ER & try to be seen? (Which I also hate, b/c anytime one goes to an ER & you're taking the pain med's I'm on...many automatically assume...you are only there to "score" some kinda drug b/c you must be an addict...You'd be surprised but I've actually had some to say those words to me...Even though I'm seen by a reputable Dr., not only drug tested monthly & pass...But they also check for the Med's you take & that you have the right amount in your system... Still, some dont care to look at your latest tests/results. Sorry to get way off base there..LOL.. Just know what I have to look forward to if I go...& many ppl will never understand how their actions make it so hard on those of us that do everything the right way..Thanks for your input & help Bryanna! I find it difficult to believe that the infection and bone loss did not show up on the xrays and scans. The purpose of those things is to check the health and dimension of the bone. So certainly any "massive" infection would have undoubtedly been picked up prior to the extractions. Sometimes, the size and areas of infection become more prevalent during the oral surgery. Perhaps that is what the oral surgeon meant about his findings at the time of surgery???

In my opinion, the decision to place implants in the jawbone at the time of extractions depends on the health of the bone.... period. If it is not healthy, in my opinion, the implants should not be placed at that time. Some surgeons will wait, others don't. Even the placement of bone graft material in unhealthy bone is not ideal.... again my opinion.

Anyplace else in the body... hips, knees, shoulders, etc... bone grafting and artificial replacements are not done until the infection is cleared. If those surgeries are rushed into, infection always prevails and the patient ends up needing additional surgeries. So why do dentists think of the mouth as being any different?? I have no idea :/

The issues that you have with your stomach... the inflammation, the bleeding.... all compromise your immune system. So any additional irritant to your immune system is going to cause you more problems than in someone who does not have your other physical issues.

May I ask....why you have inflammation around the staples that were used for your gastric bypass? Could you be allergic to the metal? I wonder about this because I believe surgical staples are titanium and most dental implants are also titanium. IF all of that is accurate then you could be having a reaction to the metal implants which could be part of what is going on right now. Does the oral surgeon know about the inflammation around the surgical staples? If not, he should be informed of that.

I too am not a big fan of the ER unless absolutely necessary, especially for dental issues. I totally understand why you would want to avoid that if possible. I think it would be wise to see the oral surgeon tomorrow and be sure to inform him of the staple issue and whatever side effects you are having with the clindamycin.

Hi Bryana,

Well, here it is 3 wks since my initial visit to the oral surgeon... I still can not eat anything other than cottage cheese or mushed food..all of which is starting to make me gag... (On the plus size, I have dropped down 2 pants sizes). I went in the following Monday, after first posting here (about the gum area around/over the implanted post coming open), and have already gone back for ANOTHER VISIT AS WELL..On the first visit, the oral surgeon said it was perfectly fine that it had opened up like that & it would actually be better for me in the long run since he wouldn't have to open it back again when having to "finish up" in a few months... I voiced concerns over food, bacteria, etc., & he said...If I had just let him finish... That's why he was going to tell me to continue rinsing w/ the blue rinse & once the sutures were removed to brush it w/ a soft bristle toothbrush... I went back on Monday 10/29/2012 & had the sutures removed & he did admit to seeing the infection but still stands firm in his belief that its best for the patient, in the long run, to have everything done at once... Even though I made sure to inform him & my general dentist that I only wanted to have the teeth extracted, let everything heal while I wore my dentures & then come back later for post placement etc... See, I had planned a trip for out of the Country for a month and yes I even informed both Dentists of my plans & I was assured they could everything done & I would be ready for my trip...HA! The 3rd planned departure date is coming fast & I don't think I'll get to go...ever! And get this... He said for me to just "sit" my dentures in on the bottom when I go out in public & that I can NOT use any kind of adhesive to hold them in place until the posts are set...So, what, I can't eat for another 2,3 or 4 months?? I'll be dead by then... I personally feel like the oral surgeon wanted to do the extractions so he could get that money on top of the money for the post placement and really didn't/ doesn't care about my well being...I can't wear the dentures b/c the general dentist that made them & does the adjustments won't make them to actually sit down onto my gums but rather they are propping up on the inside, under my tongue & the outside of the gums more so on my cheek which put pressure on the good side that wasnt open & started to pull it open, so I just took them out...W/out adhesive, I can't eat b/c they won't stay in place..heck, I can't even talk w/out them falling out... I feel like I was taken advantage of b/c these 2 Dentists refer their patients to each other for this kind of work & if you use the 2 of them, they give you a "discounted" price...
As far as my stomach, I have reflux and take many med's so they feel that's why it stays irritated all over (& where the gastric bypass was done is from my understanding just more susceptible..??) I have seasonal allergies & I am allergic to cats, dogs, fragrances, pollen, and the only time I have trouble w/ metal is jewelry...if its not sterling silver, gold or hypo-allergenic...then I break out w/ a rash & itch... Not sure about the staples, never even thought about it.... I have thought about an MRI...an the airport... LOL ...It seems to be getting better at a much faster rate since the sutures were removed!! Thanks SO much for your time with this whole mess!! I really appreciate it!! Hope you have a great weekend!!

Hi Christy,

I feel that you were not properly informed from the beginning.... I'm sorry about that.... unfortunately this is all too common a problem.

The denture cannot sit on the surgical site because it will interfere with the healing. How are you suppose to eat?? That is a major challenge for sure. Soft foods... get creative.... make soups, smoothies, juice veggies.... things like that!

It is imperative that you tell your dentist that you have allergies to metals so he can work with the dental laboratory when fabricating your find prosthesis. The same reaction you get with certain types of jewelry, you can get elsewhere in your body, even in your mouth. This could cause a chronic inflammation in your jawbone.
