Default In extreme pain. Please help.

I've been through a very tough past month and it would be very much appreciated if you please have a moment to read my situation.

I am a medical student in my 3rd year. I came back home for winter holidays to spend with my mother. On the 23rd I had incredible pain in my right lower premolars. Due to it being vacation time my dentist was closed along with everything else. Waited in a&e at whipps cross hospital and they sent me home with amoxicillin and told me to take it for 7days. The pain kept getting worse and worse.

We found a dentist who was open on the 27th. Got X-rays done which revealed a dental abscess which had grown under a root canal I did over 5 years ago. One root was left unfilled which resulted in the abscess. I was admitted in royal London hospital to have the tooth removed. By the 5th jan the tooth was removed and I was on metronidazole and amoxicillin for the bacterial infection. I was sent home the following day after my tooth was removed. Since is critical pain. I was given mophine and tramadol for painkillers which honestly was relief for maximum 3 hours and then the pain was unbearable.

On the 7th all my gums all over my mouth were swollen, red, and very patchy. They almost were coming down and covering my teeth. I went to my dentist who was very worried and urgently referred me to hospital again. This time I didn't want to go royal London as I wasn't very pleased with how I was treated before. So my mother took me to queens hospital. My dentist said it cannot be ANUG as my inter dental papilla was still intact. But at Queens hospital they kept insisting it was ANUG. I was admitted in hospital there. Was put on IV antibiotics (metro and augmentin) and was continued given morphine and tramadol for the pain.

Nothing worked. My gums were extremely painful and still the same. Blood tests revealed my leukocyte count was low. Other than that blood tests were normal. An ultrasound was done neck downwards which revealed some inflamed nodes but the doctors said it was nothing to worry about. They eventually have me bethmethasone mouthwash. I had developed rock solid lumps on my cheeks, the right side being bigger than the left. The doctor said that she will do an ultrasound of that in a weeks time if they didn't disappear. And discharged me as they said it will be a slow recovery and they can't keep me In Hospital for that long. So I went home. Still in pain. I was prescribed oro-morph for home and tramadol too.

I kept using this steroidal mouthwash and kept brushing my teeth no matter how painful it was. After a week, my gums got better. The swelling is still there but dramatically decreased. And my teeth were more visible. I'm now waiting for the ultrasound to be done on the lumps on my cheek. They said if needed they will do a FNA too. Ultrasound will be done on the 7th Feb.
all throughout this, from when I was admitted in royal London (4th Jan-6th) till now my lower lip and chin are numb. Mostly the right side. To every doctor in every hospital I went to and have seen now I keep complaining about this numbness. But no one knows why it is. When my tooth was removed due to the abscess they gave me an anaesthetic.. I'm very concerned about this. As now I think the pain is from the numbness of my chin and lower lip and the teeth that are painful on the right might be referred pain.

I went to two private hospitals. And saw maxfax consultants. One has started me on norytriptalline as he thinks it's neuropathic. Im aware the drug will take time to work. And the second consultant I just came home from is saying it might be osteomyelitis. I'm ever so worried. The pain is unbearable. It's almost electric. Pins and needles type.
Also for the past two weeks I have been suffering from nose bleeds. It's so bad that it clogs up my nose and I have to breath through my mouth which dries up when I sleep so disrupts my sleep. I haven't slept in the past 5 days.
I've been to so many doctors. So many hospital. Private dentists. I now do not know what to do. The pain is still there but no way near as awful as It was when it all started. And the numbness is still there in my lower lip and chin.
Please advice me in what to do. Or if you know anyone who is a very good maxillofacial doctor. I am very hopeless now. Please could you guide me in what to do.

Hi Debra,

I am in the dental profession and will try to help as much as I can.

My first thought is.... do you have periodontal disease or chronic gingivitis? Do you have regular dental care? Are you on medications that compromise your immune system and/or contribute to inflamed gums?

Regarding the root canaled tooth.... you need to be aware that the root canaled tooth was infected prior to the "abscess" that developed at the root of the tooth. The issue with the root canal procedure being incomplete is truly irrelevant. All teeth have millions of tiny canals that contain nerves and are not accessible. The only canals that are treated during a root canal are those that are accessible. There is no dental procedure or medication that can make an infected tooth healthy again. So it is only a matter of time before the the bacteria spreads from the tooth into the jawbone. The longer this tooth is in the mouth, the more proliferated the infection. Symptoms of the infection can be mild, intermittent or basically non existent for months or years but the infection is brewing the whole entire time.

It is not unusual to have a lot of post op pain when a badly infected tooth is removed because the are of bone that is also infected needs to be removed along with the tooth. Sometimes the infection has spread to other teeth and unfortunately the dentist does not recognize this and only removes the one tooth leaving the patient in a compromising situation. Meaning active infection is still present in an area that just had surgery.

Your description of the inflammation of the gum tissue and how it was growing over your teeth may have been due to ANUG and/or gingivitis (as there are many different forms of this) in addition to an allergic reaction to the meds you were given. Also, if your mouth is not healthy as in perio disease then a bone abscess can exacerbate the already existing inflammatory condition. Keep in mind that this tooth has been infected for several years so that bacteria has been brewing the whole time. Oral surgery stirs up or stimulates bacteria, inflammation and causes trauma. So you may have been a host to complications following the extraction of this tooth because of other pre existing (bacterial) conditions. Did I explain that okay??


The numbness in your chin, etc.... could be related to any number of things as I said you have a complicated situation. The hard in your cheeks could be a form of angioedema from infection, medications, trauma....... etc.

What did the ultrasound show??

Double check for infection.
But be careful about removing teeth, unless there is clear reason to do so.
I do have neuralgia after dental work. Many people end up with un-needed dental work, including extractions, simply because the dentist is fishing around looking for where the pain is coming from.
Has the norytriptilline helped? I'm on a similar med and it did help my nerve pain a little.
Hopefully, if you have an infection or some other dental issue, it will be discovered soon and you will get good and proper treatment and relief.

Just to add it sounds (to me) like some kind of nerve damage maybe neuralgia and an infection.There are supposed to be a few very good pain clinics dealing with pain after dental implants especially I think one is in London the other associated with Oxfordased neuropathy associations and Pain University.Tomorrow afernoon I can look them up for you they sound pretty good. Or ask -call -the Neuropathy Trust. Perhaps google chronic orofacial pain.Elaine55
