Default Sinus and jaw pain weeks after extraction?

Hello,
I'm new here, and joined after seeing how professional and helpful this forum is. I hope to be posting in the other forums regularly

What brings me here today is this dental issue I am having. On April 9th, I had an erupted upper wisdom tooth extracted. Extraction went well, took only five minutes, local only, etc. I was sent home without an prescription painkillers, this is how well it went. Oral surgeon said I would only need advil. No antibiotics, since there was no infection and the OS felt one wouldn't occur.

As it's healing, it starts getting worse instead of better. Around day 4, it constantly throbs, and hurts all the time. Not excruciating, but very noticeable. I do back, and I developed a dry socket. The OS clean the socket out, then packed it with the dissolving paste and whatever else they use. She prescribed antibiotics since the food trapped in there had caused a small infection, nothing she was worried about.

I take the antibiotics fully, finished them, while letting the socket heal. The pain is much better, and it is only sore and throbs only if I chew a bit too much. Last Friday, I go in to check in on it, and the OS says it's healing perfectly, and the area is very clean as well. (no food, no sign of infection).

Then things go bad. Saturday it throbs, feeling like the dry socket came back. For the next few days, it is sore and throbbing, and the socket is sometimes itchy. Pain level would be a three, not really painful, but annoying. I have no swelling, no redness, no fever, the area isn't warm. Looking at the socket, the gums are pink and healthy around it, no swelling, no bad taste or smell. I can see the new tissue filling bottom up, reddish/pinkish, the socket is starting to close.

Now, the throbbing is gone, but it is still sore. I know have a headache that moves from the sinus's to my temples. My sinus's are sore, and I can feel sinus pressure all the time. My throat is sore, and I can feel post-nasal drip as well. What is happening here? Is this a sinus infection? Am I just getting a cold and being paranoid? Is there a chance of an infection in the socket? Or is this just healing pain? It's also gotten very humid and warm here, could the barometric pressure be messing with me? (I am sensitive to weather changes generally).

I would love to hear some thoughts and opinions, and what I can do in the meanwhile to help it.

Hi eakorneinko,

Sometimes the roots of an upper wisdom tooth are very close to the sinus and sometimes the root can actually perforate the sinus. So when the tooth is removed there would be a hole through to the sinus. If it's not a perforation, there could still be inflammation in that sinus area from the oral surgery and with the weather change the sinus could be acting up.

It is probably wise to call the oral surgeon, tell her your symptoms and ask her if there was any chance of a sinus perforation caused by the root of the tooth or from extracting the tooth. Get her opinion on that as she was the one who extracted it and would know if the sinus was involved.

So long as there is no infection and the site is healing well, sinus perforations usually heal closed without any intervention. But it would be best to speak to the oral surgeon.
