The endodontist says this looks fine because there is no obvious cyst or massive area of pus around the tooth. This tooth looks like it had a root canal and to a dentist this is the way it is suppose to look.

Because you are complaining about pain in or near this tooth, the only option, other than extraction, that he knows to offer you is to do another apico. He is right that the MTA is irritating to the bone, but any material that is jammed in the bone is going to be irritating.... irrelevant of what that material is. Re surgerizing and exchanging one material for another does not alter the anatomy of this tooth or alter the anatomy of the bundles of nerves and vessels that surround this tooth. The problem you have is not about the filling material.... it is the tooth and the lack of vitality and all that goes with that.
