Default Help please - need advice, which teeth look problematic on x-ray?

Hi All,
Dear Bryanna,
You helped me through a never-ending nightmare with a sinus-opening and botched oral surgeries 2 years ago, after I had 2 root-canaled teeth removed to help my body battle post-Ramsay Hunt Syndrome problems. That issue is still ongoing and I've given up hope, but it doesn't matter - accepted.
Your support helped me through the first and worst year for which I am grateful every day.
Help someone and they will remember you when they need help again
Would you help me again please?
The attached x-ray was taken 2 weeks ago. Last week the filling was changed in the left 7 (the side with the one remaining root-canaled tooth), even though I feel there is a problem with left 6 - or maybe both?
According to the dentist left 6 is ok. Do you agree?
Also, I will have the one root-canaled tooth removed as I would like to not have any left in my mouth - I was told it does not look bad but am I crazy for really wanting it out? Would you leave it in?
On the same side the upper 4, the last before the gap, does not react to the cold test at all.
Is it surely dead then? I will never have a root canal again so it will have to go.
Would you get an implant (non-metal) in there (or anywhere ever?) if you were me or do you think a denture will do and not damage n.3?
One more question please: do you see anything worrysome on the other side, especially on upper 6 and 8?
Does anything else look 'off' to you on the x-ray?

I'm sorry! First dental treatment since the ordeal, I am scared and trust noone.

Dear Bryanna, Dear All,

There's been some developments since the original post - I decided to see another professional who also has a very good name. He will extract the one remaining RC tooth tomorrow evening (I'm so very happy about it) and if I will still feel that no. 6 just beside it hurts, they will open it next week, fix if possible without RC but remove it if not (that's the one with the huge filling, sensitive to cold and hurts a little but looks OK on CBCT. Dentist thinks it might be the RC beside it in fact). I feel better about this doctor and his team, even though I don't think he's an oral surgeon: dentist and implantologist, but he teaches dentists and has a very good reputation. The place is called 'dentist and surgery' but in his resume it only says dentist. If he does implants he must be OK with extractions as well isn't it. Rather a very good dentist than an average OS? Dunno.

There is no problem with extracting a tooth tomorrow and the one next to it in a week's time, or is there? They would have said something wouldn't they?

They said the upper left 4 has healthy bone around it, there is no infection anywhere and that the fact that it does not react to the cold test does not necessarily mean that the tooth is not vital. It never hurt and I hope that they are right.
What if it is not vital and stays in? Will there be some warnings as it starts damaging its surroundings or only late into it?

I'm sorry, just would like to get it right this time and have rather less than more problems in the end

Default NTI-tss and other suggestions

I am turning this into a temp blog for myself - it helps me doing it, just for another week or so.
I met two better-than-any-so-far dentists yesterday. I am looking for the unfortunate one who will be the One, poor soul.
Truly amazed - the first one referred me to a very good oral surgeon as he thinks what I have been thinking for years but have not mentioned it to him or to anyone lately as it was always dismissed : that something sinister is going on with the bone on the upper left side. He also thinks that the healing of the extraction sites from two years ago and the remaining teeth around them give reason for concern - again, I do know, there are things poking out, it hurts and just does not feel right, but it was dismissed by each and every one and there is nothing I can do about it without a doctor. He was great, calm, he said that RC can never be 'pure' but also said that given the state of the one left RC tooth on the bottom left, I should not have it extracted as it gives no reason for concern - he said if I were to keep it he would 'bleach' the gum around it but I was to have it extracted later in the day so we didn't do it.
He also said that it is made sound easy but in fact replacement is anything but easy. He suggested I had an allergy test for everything including RC fillers and materials of possible replacements including dentures. I trusted him although this bleach thing didn't add up but I can tell he wants to do good and is open enough to not only see what he feels comfortable with.

Next up, a few hours later arrived for the extraction of the RC tooth bottom left 5 and the dentist made me sign the papers, I sat down, then he started to talk and ask and listen. He said I am right about this tooth poisoning me, I'm just wrong about the reason - he said there was a cavity on the side of the tooth which he thinks caused the gum problems as well around it, while the RC () itself and the bone looks completely fine. He also said that it is true, RC can never be 'perfect' because of the structure of the tooth (never heard a dentist say that and now 2 in a day!) and that somewhere in France they started using a new equipment that pushes steam? deep into the tooth somehow and then does the same with the filling - making it sound a bit better / more effective?
So he asked me to give him one trial with the tooth, we will know in 2 weeks whether it was only the cavity by the side of the tooth.
I was amazed by him actually wanting to do something so of course said yes (also told him that I keep the right to have it out if I need to but also didn't accept his offer to do this for free). He worked on it, said it was kind of deep but fixed now and we will see.
Funnily, gum already feels better, is that possible at all? There was a definite relief, even though the RC might still be doing its job, it is less of a trouble now for sure. And I would love for it to stay just for a little while longer - not ready.
He said he thinks my jaw is out of place and some of the pain is caused by me clenching the teeth at night (might be, but hard to tell with all the pain left behind by Ramsay H?unt) and wants to make me an NTI-tss - he's an expert in it so that makes me worry that he sees the need of it everywhere. He wants to have it done free of charge and I should pay only if I experience a positive effect within a month as he is convinced I will. Firstly I can not have have it done for free, I just can't, and I am worried that using it might just cause new problems eventually. Or - knowing my luck - immediate death by choking. I admire him for trying to help but I can not waste GBP250 for something to stand on a shelf. Or should I reward him with it for doing something that might as well be just normal from professionals? I might...
So here I am right now. Next week change of fillings where there's cavity by this last dentists colleague (as he does not normally deal with minor issues), maybe mouth guard, allergy tests, then OS on friday. He wrote an article about facial palsy as well so he might be great for me all in all.
This year I found THE neurologist (expert in RHS and PHN, can not help much but he knows what is what), THE ENT (stopped the once a month ear infections that I had since RHS), THE gynaecologist (operated and solved a long-term issue that started post RHS).
I am doing something right, however weird it might sound.

Just want to be as healthy as I probably can, deal with everything that can be made better and live with the rest in peace.
And have my jaw and teeth not poisoning me any further. I wonder how many health issues are brought on by our teeth in the end really. I wonder if the two upper RC teeth caused the VZV reactivation 3 years ago. Not that it matters now these icons

Hi Elna,

I scanned your post in a hurry, so i'm not sure how helpful this will be. First, I would remove teeth as a LAST resort. A good endodontist can help heal the infection and save the tooth; however, it seems like you have a medical condition that might warrant removal. Get a second opinion from a biodentist.

Dental implants are really a pain, but are the second best option if you can't keep the tooth. Ceramic is a must if you go this route.

The night guard didn't help me. I had jaw pain supposedly from clenching but i think it was a result of all the dental work i had. I recommend acupuncture treatment for TMJ;this helped me big time. I don't believe all acupuncturist practice the same though. The guy who treated me is Korean **and he followed a chart of some kind. He was excellent but a bit pricey ($60 a session, but it was well worth the money!). In contrast, some acupuncturist just stick needles everywhere and believe the healing extend throughout the body; i would stay away from this kind of practitioner.

Hi Elna, sorry I can't give any advice. I was just reading through your previous posts yeasterday about the ordeal you went through a couple of years ago. I am in London as well, what was the outcome of your sinus communication. Did it heal on its own?

Hi David,

This was telepathy - I read though your story yesterday and wanted to reply but decided against, as I didn't want my story to scare you.
Especially as I remember the terror of not knowing what will happen or what to do...
I hope someone will reply and give medical advice before next week as I would give it longer than 2 weeks before letting anyone touch it but it depends on so many factors so this is just how I feel now that I wish I did.
To answer your question, it took several month and there is no air or liquid movement directly, but sg is not right in the area. But! since you read the story you know that I had a health condition that didn't help and 2 operations within 3 days so it was savaged really.
It sounds really good that 1 week and there's no liquid flow for you. I hope the doc will give you good news next week!!!
I found it extremely difficult to find help in London, I'm glad that Portuguese OS sounds good! In the end I got into University College Hospital where I felt in good hands finally, we agreed to give things some more time then I left the UK for a while so didn't go back (yet).
Please keep posting what happens. Best of luck! Soon it will be history I hope

Oh no.
Light burning sensation started on the tongue and in the mouth in general since the morning after the dentist treating the RC tooth with an apparently deep cavity and this morning I woke up with a horrid pain in my throat, barely able to swallow or talk really. I have many things going on since RHS but never a sore throat like this for many months now.
I'm worried sick that by being so stupid, I now have all the bacteria running around in my body doing whatever damage again.
Wish I had the tooth out and not be so weak to change my mind.

Default Are muscles causing the pain?

Mea culpa. ENT had it checked and there was no bacteria in my throat at all, nothing, so it must have just been an opportunistic cold. Good news, I am a bit worried about all coccus-es in general.

During the cold I was coughing really hard for a couple of days and it resulted in unbearable pain somewhere deep in my upper left side of the face. Would have panicked if I wasn't to see the OS today anyway.
Which I did. He wasn't able to open the CBCT, but just by the X-ray he said that everything looks jolly good, the only thing he thinks causes me pain in the upper left or even both sides is that the muscles are sticking to I do not know what and are somehow pulling the vulnerable thin part of the bones.
He said this is unlikely to cause the tinnitus and other weird symptoms on the left side.
He said this thing with the muscles can be very painful and cause sudden strong pain just what I was experiencing the past few days. He said my jaw was complately fine and there are two options: treat the pain itself or the cause, which means fixing the gaps on the upper left part. With implants... which I don't think I will have.
He sent me to the hospital he works at to see a good ENT and have another ct taken and confirm the sinuses are OK and we can take it from there.

I hope he is right.
Anyhow. 2 months in and nothing is sorted, I'm really not good at this. RC tooth is still in the jaw, I must admit it the gum is perfectly fine around it now and have no symptoms in the area - the bottom left 6 is hurting though.
I'll stop everything now again, have to go away for two months, hope nothing will blow up in my mouth by then and will start again in September.
So tired. If my head would be a limb, with all the tooth and face and ramsay and neuralgia and ear troubled I would have have it chopped off already.
