Default still pain after 2 months of wisdom tooth extraction

Merry Christmas! I am looking for some advice and help here as my dental problem has been bugging me for almost 2 months.

I got my right side upper wisdom tooth extracted end of Oct. The procedure
was smooth I think although the surgeon did push the tooth hard a couple times.
I felt fine for the first 2 days after the surgery. On day 3 I begun to feel like some gum throbbing pain (5-7/10) for the rest of the week.

Week 2 to week 4, the throbbing pain on the gum seemed to subside however I have pain/pressure like on the temple and ear. Went back to Dr, said its normal.

Week 5, the ear and temple pain has become less, although there was sometimes pressure on the ear but I didn't feel the pain any more. However, my extraction site was hurting again, more like a throbbing pain and also the gum surrounding it has a inflaming pain feeling. The pain was actually quite bad for a couple days
and after I went to see the Dr he said there might be a small infection and gave me 7 days of amoxicillin. He also did an X ray and 3D scan to make sure nothing else is going on.

Week 6, i stared to have some sinus symptoms, I remember a few days after the tooth was extracted one side my nose was congested. However, this time I feel like the other side was blocked as well. I felt pressure coming up to my face and forehead, and I started to have post nasal drips. Also, sometimes when I feel the drips was at the throat my extraction site would taste like salty or whatever taste that like the drop. I am suspecting I may have an sinus communication. I went back to the doctor and he said the x ray and scan looks fine, when I mentioned I had the sinus symptoms and salty taste then he said there may be a small communication. Then he asked me if I had liquid drip from nose, I said no, and we had the blow test, no air seems coming out from my socket. So he changed his mind that saying a communication is not likely. And for my extraction site pain it may just need some more time for it to heal.

Now, I still feel the salty taste once a while, maybe a couple times a day last like 20 mins each? I feel like if there was no post drip then I don't feel the taste, or there is some time I had post drip but no feeling of salty taste. I am not sure my sinus symptoms are related to tooth as I did have post nasal drip in my life before. For the sinus pressure I took sinus pills and it seemed off. However, what bothers me now is the salty taste, am I having a sinus communication? Even though I had no liquid drain and air leakage from nose, maybe the hole was too small even on the scan it's hard to find? If I did have the hole then is my slow recovery and extraction site pain related to this? It has been 2 months now if there was a hole there, does that mean the hole failed to heal on its own? If the hole was that small how will the Dr locate it and even tried to repair it?

Hi Leon,

Did an oral surgeon or a general dentist extract the wisdom tooth?

Why was the tooth removed?

Was the tooth decayed?

Was there an infection associated with this tooth?

Did the dentist mention that the root of the tooth may be in close proximity to the sinus? That is a common occurrence.

Was the tooth removed in it's entirety or was a piece broken off and left in the area?

Your symptoms mimic either a sinus communication, a retained piece of tooth and/or an infection.

It is best to consult with an oral surgeon. If the treating dentist is an oral surgeon than get a second opinion. You will need a copy of your pre and post op xrays, copy of the surgical report and the scan for the second opinion.

Hi Bryana, thanks for the reply. The surgery was done under an oral surgeon, because it was impacted and it was causing pressure to the tooth beside it. No decayed but the tooth did cut the gum out when it grew thats why it drew my attention and I decided to have it removed. The tooth was removed a whole piece I guess as after the pain I had x ray with my general dentist and she found nothing left in the area. As you said, I am going to have a second opinion from a different oral surgeon. My original oral surgeon did mention if my symptoms persist he may need to open the wound again to find out what's going on. Do you think it is a good idea?

Default please help

Hi. It's been almost 2 months also since extraction of my upper left wisdom tooth (difficult extraction - 30 minutes and caused sinus perforation
he stiched the gums also with some of my cheek)
One week later I was on antibiotics for 10 days and a second batch of antibiotics almost back to back due to infection- yello "pus" from
extraction site and throat and a few times from nose sometimes with bad smell.
Now off antibiotics for 2 weeks. not sure if still infection as only feeling something running back of my throat (don't know colour but no smell) and maybe sometimes salty fluid from extraction site. My OS said to wait 2 weeks and come back and he would probably reopen to clean up the sinus
maybe food stuck in there that my body cannot evacuate) but he told me that he would not require a CT scan first (when I asked)
I would like to know if this is best option and if CT scan would not be advisable before reopening. Are there any alternatives?

It may be helpful to have a 3D dental scan to help evaluate that site and the sinus area irrelevant of whether the oral surgeon goes back in to clean it out or not. The course of treatment would depend on the findings of the 3D dental scan.
