I would say the Percocet... maybe dose related.
Lyrica is sedating, and so is Ultram, and when you add the potent Percocet, it might be too much. After two months she should not need the Percocet so much anyway. While needing the major pain relief, she might have been able to drop the Ultram, (taper off) while using the Percocet. They both work on mu receptors, but Percocet is much stronger. Ultram and Lyrica should be tapered off if discontinued or reduced. I'd also get the thyroid checked. Some older patients, get temporarily hypo when they have trauma or surgery. Low thyroid would make her weaker etc. These are just some ideas.
