Post Contemplating Seeking Accommodations in College

I have recently been diagnosed with Executive Function Disorder (due to perceived damage from neurosurgery to remove 3 Arteriovenous Malformations from my left frontal lobe almost 6 months ago), after discussing my difficulties in prioritizing school work and getting things done with my neurologist.

I wrote about it on the ADHD sub forum hoping for some advice in accommodating/working around the issues (without use of medications meant for people with ADHD), but it seems as though not too many people frequent the board (neurotalk.psychcentral.com/thread170320.html).

I am wondering whether I should perhaps request accommodations (maybe extensions on due dates when necessary) from my university for the upcoming semester, but though I have a friend or two who has gone through the process, I do not keep in contact with them, so I have no idea how to go through it.

I think people who are diagnosed with ADHD do apply for accommodations, but though I have symptoms that are very similar to ADHD, I do not technically have ADHD (it is something more related to damage in the executive functioning). Are accommodations classified by disorders (such as this and this for people with ADHD) or in need-based ways (she has trouble focusing, so she will get a tutor for this subject)?

Does anyone know anything about obtaining accommodations in college? (Does it go onto your "record"?)
