Default Doctor permission for rehabilitation

What does anyone think of a doctor, specializing in what a patient is having an issue with, not giving permission to go to a rehabilitation hospital to improve their function?

This doctor requests at home physical therapy. The physical therapist recommends two to three weeks of physical and occupational therapy at the rehabilitation hospital for two to three hours a day (general on the weekdays) of intensive therapy versus a half hour to hour two or three times a week at home for nine weeks... and if helping another nine weeks.

One or another doctor should give permission.
