Default Ca Labor Code On Transportation

Does anyone have the link where I can get info on transportation to and from a physicians office or hospital?

Is there such a thing as a 20-25 mile radius changing my PTP?

To a hospital for an approved procedure?

Can the insurance co legally deny me transportation?

I have been online waaay too long researching and the pain so so BAD
